Not Enrolled in Medicaid
Beginning July 1, 2003, Georgia’s Nursing Home Provider Fee Act required that all of the state's nursing homes pay a fee based on the number of patient days of service provided. The fee is due from all licensed nursing homes except federal and state facilities, and all patient days other than those paid by the Medicare program are included in the fee calculation. The fee amount payable for each patient day may vary based on a facility’s number or percentage of medically indigent patients served.
For nursing homes NOT enrolled in Medicaid.

Provider Fee Payments
This file includes the forms needed to set up the electronic funds transfer for payment of provider fee payments. The forms may also be used to make changes to facility name, home office name, contact person, and bank information related to provider fee payments (only).

Send the completed Excel data file as an e-mail attachment to the department at email@example.com. Reports must be submitted by the 30th day following the end of the reported quarter.
