Freestanding Cost Report changes

• Page i: Added lines to report nursing facility's physical address, administrator contact information and Federal ID and ICF ID.
• Schedule B-2: Added line separately identify General Liability and Professional Liability insurance.
• Consolidated the reporting of Barber & Beauty, Religious Services, and Inservice Training expenses. They should be reported on lines 126 to 128.
• Schedule B-4: Added standard lines for Benefit Allocations for each cost center.
• Schedule G-1: Added line for Central Supply position.
