Home Health Services
Overview
The Department of Community Health (DCH, Department) reimburses home health agencies (HHA) who provide skilled nursing, physical therapy, occupational therapy, speech therapy and other skilled services to eligible Georgia Medicaid members.
Home Health Agency Classification
Home Health Agencies are classified into one of the following categories:
- Freestanding
- Urban
- Rural
- Hospital-Based
Medicaid Data Form and Medicaid Cost Report
Georgia Medicaid HHA must submit an electronic copy of their completed Medicaid Data form and Medicare Cost Report to DCH. Data forms and cost reports are due to the Department 150 days after the end of the HHA fiscal year end. Parent Office Cost Data Form
A parent office with multiple Georgia home health agency locations must complete a Medicaid Data Form for each home health agency. The parent office must complete the Parent Office Cost Data Form.

Payment Methodology
Each home health agency is reimbursed a specific rate per visit for covered services. For more information on how home health reimbursement rates are calculated, refer to Provider Portal, Provider Information, Provider Manuals, Home Health Services Chapter 1000.
