Medicaid and PeachCare for Kids®

  • Am I eligible for Medicaid or PeachCare for Kids® in Georgia?

  • How do I apply for Medicaid/PeachCare for Kids®?

    You can submit an application in one of several ways:

    • Online at www.gateway.ga.gov
    • In person at your local DFCS office (please review new office protocols due to COVID-19)
    • By phone at 1-877-423-4746
  • Once approved for Medicaid/PeachCare for Kids®, how do I report changes to my case (address, income, etc.)?

    You can submit changes to your Medicaid/PeachCare for Kids® case in one of several ways:

    • Online at www.gateway.ga.gov
    • In person at your local DFCS office (please review new office protocols due to COVID-19)

  • How do I request a copy of my Medicaid/PeachCare for Kids® ID card?

    To request a copy of your Medicaid or PeachCare for Kids® ID card, please call 1-866-211-0950 and choose option ‘3’.

  • How do I locate a provider who accepts Medicaid/PeachCare for Kids®?

    You can search for providers accepting Georgia Medicaid/PeachCare for Kids® here or by calling our Powerline at 1-800-822-2539.

  • Who should I contact if I have questions about the benefits or services offered through my Medicaid Care Management Organization (CMO)?

    To inquire about covered benefits and services, please contact your CMO directly.

  • As a Medicaid/PeachCare for Kids® member, how do I schedule Non-Emergency Medical Transportation (NEMT) to/from my medical appointments?

    To schedule transportation, members must contact the NEMT Broker serving their area. For additional information, including a listing of counties covered by each broker and the corresponding phone numbers, please visit: here.

  • Who do I contact with questions, comments or complaints about a Non-Emergency Medical Transportation Provider?

    Please call the Member Call Center at 1-866-211-0950.

  • How do I become a Medicaid provider?

Healthcare Facility Regulation

  • How do I file a complaint against a healthcare facility (including hospitals, nursing homes or other long-term care facilities)?

    You can file a complaint online here or by calling 1-800-878-6442.

  • How do I apply for licensure as a healthcare facility?

    Application forms can be found here.

  • How do I find a licensed healthcare facility or review their inspection reports?

    GaMap2Care is an interactive website that allows users to search for facilities by type and location. It also provides access to inspection reports for licensed facilities.

State Health Benefit Plan

  • How do I become a member of the State Health Benefit Plan (SHBP)?

    To become a member of SHBP, you must be a Georgia state employee, public school teacher, non-certificated employee, or retiree of any of the above whose employer participates in the SHBP. More details can be found here.

  • How do I get answers to questions about my covered benefits?

    Contact SHBP Member Services via email at [email protected] or by phone at 1-800-610-1863.

  • Where can I view rates for the different plan options?

    SHBP member rates for both active employees and retirees can be found here.

  • Where do I go to enroll in my SHBP benefits?

    The enrollment portal can be found here.

  • How do I appeal an SHBP decision?

    To appeal a medical claim, contact your Medical Claims Administrator at the number listed on the back of your insurance card.
