Rural Health Transformation Program Application Has Been Submitted

The Georgia Department of Community Health is excited to announce that Georgia’s application for the federal Rural Health Transformation (RHT) Program has been submitted.

After months of public input, including four townhalls and over 150 submitted ideas, the application compiled numerous proposals with the intent of creating lasting, transformative change to the rural health ecosystem in Georgia. This is the beginning of the process for the RHT Program grant. Further details on what entities may be eligible for each opportunity and application processes will be released at a later date. 

The Rural Health Transformation Program page, linked here, will continue to serve as the primary source of information sharing throughout this grant. CMS is set to notify states of their award by December 31, 2025. All official communications to the public regarding final awards will be posted to this page. 

Resources will be regularly updated on the DCH website and currently include the submitted application for public review, as well as additional letters of support, Frequently Asked Questions (FAQs) and other helpful information.
