Georgia’s Application to the RHT Program
DCH has worked with CMS and our partners to update our GREAT Health Program proposal to match the award amount the State received. Please see the below Year 1 Budget and updated Project Narrative to reflect our award and necessary adjustments to ensure the Program’s success. Please note these have been submitted to CMS but are awaiting final approval, which allows Georgia to begin funding our initiatives.
Additionally, as it relates to Initiative 1 and value-based care model participation, please see the list of hospitals that meet the eligibility criteria outlined in the State’s application and have received an invitation to submit a Letter of Intent to participate in our AHEAD Model pre-implementation period.
