Effective July 1, 2009, the Fair Rental Value (FRV) System replaced the Dodge Index in the Property and Related Net Per Diem reimbursement methodology. Before a bed addition or renovation project is started, a Certificate of Need Request for Determination Form must be completed and submitted to the Department's General Counsel Division. Once the project has been completed, the Initial Start Up And Fair Rental Value System Reimbursement (FRVS) Update Request Form should be completed and submitted to Nursing Home Reimbursement Services along with all supporting documentation.
