Find Facility/File Complaint

Finding information about a specific facility or facility type in the state is a simple matter of using our Find a Facility search tool.

Facilities are displayed in green if inspection reports about them are online. If there are other reports available online, you will see a "YES" under the column headed "Inspection Reports Available." However, Heath Information Portability and Accountability Act (HIPAA) rules prevent some reports from being placed online.
