Wellness 2017 - BlueCross BlueShield of Georgia (BCBSGA) and UnitedHealthcare Non-Medicare Advantage Members & Covered Spouses

The State Health Benefit Plan (SHBP) is excited to continue with our Wellness partner, Healthways, to provide non-Medicare Advantage (MA) plan members who elected Blue Cross and Blue Shield of Georgia (BCBSGa) and UnitedHealthcare with comprehensive well-being resources and incentive programs. These members and covered spouses will have access to a variety of Healthways’ tools and services such as the Well-Being Assessment®, Well-Being Connect (a guided online website), well-being incentives, well-being coaching including tobacco cessation coaching, biometric screenings, onsite activities, sweepstakes, challenges and much more. SHBP non-MA plan members and spouses covered under BCBSGa and UnitedHealthcare can each earn up to 480 well-being incentive credits just for participating and engaging in healthy activities – that is a family total of up to 960 well-being incentive credits. These well-being incentive credits can be used for eligible medical and pharmacy health care expenses. For detailed information about the BCBSGa and UnitedHealthcare Plan Options, review the SHBP Decision Guides click here and Summary Plan Descriptions click here. The 2017 action-based well-being incentive credits will be earned as the healthy action is completed and deposited in the member’s BCBSGa HRA Account or BCBSGa HMO MyIncentive Account or in their UnitedHealthcare HMO or HDHP Health Incentive Account monthly. Note:  Participation in the 2017 Healthways Well-Being Program is not applicable to Kaiser Permanente or the Medicare Advantage Plan Options. However, regardless of the Plan Option you selected (excluding TRICARE Supplement), all unused well-being incentive credits earned while participating in SHBP’s 2016 wellness program will now rollover if you remained enrolled in the same Plan Option and vendor or changed to another Plan Option and/or vendor during Open Enrollment (OE).  Please see the section below entitled “Well-Being Incentive Credit Rollover Between Plan Options and Vendors”.

2017 Well-Being Incentive Credits

To earn these well-being incentive credits, complete the following actions between January 1, 2017 and December 15, 2017:

1. Assess your health

Complete your 2017 Healthways Well-Being Assessment® (WBA), a confidential, online questionnaire that will take about 20 minutes. Please note:  the Well-Being Assessment must be completed before well-being incentive credits can be earned.

2. Know your numbers

Complete a 2017 biometric screening and submit your results (body mass index, blood pressure, cholesterol, glucose). The biometric screening must be completed at a SHBP-sponsored biometric screening event or by your physician in their office and your results submitted appropriately on the 2017 Physician Screening Form. By completing both actions, “Assess your health” and “Know your numbers,” BCBSGa and UnitedHealthcare plan members and covered spouses can each earn up to 240 well-being incentive credits. Please note:  the Well-Being Assessment must be completed before well-being incentive credits can be earned.

3. Take action

It’s your choice! Complete the coaching pathway, online pathway, or a combination of both. By completing the healthy actions outlined in “Take action,” BCBSGa and UnitedHealthcare plan members and covered spouses can each earn up to 240 well-being incentive credits. Please note:  the Well-Being Assessment must be completed before well-being incentive credits can be earned.

Coaching Pathway

  • Complete your WBA and;
  • Actively engage in telephonic coaching and earn 60 well-being incentive credits for completing one (1) coaching call in a calendar month up to four (4) times in a Plan Year for a maximum of 240 well-being incentive credits NOTE: Only one call in a calendar month qualifies for the 60 well-being incentive credits

Online Pathway

  • Complete your WBA and;
  • Record 5 online well-being activities using the same tracker within a calendar month and earn 40 well-being incentive credits. You can earn these rewards up to 6 times. A sample online activity would be to track your exercise five times within a calendar month.

The total 2017 well-being incentive credits earned by completing the above steps may be up to a maximum of 480 for each member and spouse, if covered. Well-being Incentive Credit Rollover Between Plan Options and Vendors

All unused well-being incentive credits earned while participating in the 2016 Healthways Well-Being Program called Be Well SHBP will now rollover in 2017 whether you remained enrolled in your current Plan Option and vendor or changed to another Plan Option and/or vendor during Open Enrollment (OE) for the 2017 Plan Year.   If you enrolled in Kaiser Permanente (KP) during OE, your unused well-being incentive credits earned while participating in the 2016 Healthways Well-Being Program will rollover into a Kaiser Permanente Rollover Account (KPRA).  However, you will be ineligible to participate in the 2017 Healthways Well-Being Program if you enroll in KP.  You will be eligible to participate in KP’s Wellness Program. 

This means no matter what Plan Option you selected during Open Enrollment for the 2017 Plan Year (excluding TRICARE Supplement), you will not have forfeited any unused well-being incentive credits you earned!

Note:  If, during Open Enrollment, you selected a different Plan Option or vendor for the 2017 Plan Year, your unused well-being incentive credits will rollover in April of 2017.  This allows 2016 well-being incentive credits to be used to pay your eligible out-of-pocket expenses for 2016 claims filed after December 31, 2016.

2017 Wellness Appeals (BCBSGa and UnitedHealthcare)

Between February 15, 2017 and January 31, 2018 you and your spouse (if covered) may appeal the total well-being incentive credits applied if the rewards are less than you believe should have been awarded to you or your spouse.
