Wellness 2015 for Blue Cross Blue Shield of Georgia (BCBSGa) and UnitedHealthcare non-Medicare Advantage members and covered spouses
The State Health Benefit Plan (SHBP) is excited to continue with our Wellness partner, Healthways, to provide non-Medicare Advantage (MA) plan members who elected Blue Cross Blue Shield of Georgia (BCBSGa) and UnitedHealthcare with comprehensive well-being resources and incentive programs. These members and covered spouses will have access to a variety of Healthways’ tools and services such as the Well-Being Assessment®, Well-Being Connect (a guided online website), well-being incentives, well-being coaching including tobacco cessation coaching, biometric screenings, onsite activities, sweepstakes, challenges and much more. SHBP non-MA plan members and spouses covered under BCBSGa and UnitedHealthcare can each earn up to 480 in well-being incentive credits just for participating and engaging in healthy activities – that is a family total of up to 960 well-being incentive credits. These well-being incentive credits can be used for eligible health care expenses. The 2015 action-based well-being incentive credits will be earned as the healthy action is completed and deposited in the member’s BCBSGa HRA Incentive Account or MyIncentive Account or their UnitedHealthcare Health Incentive Account monthly. 2015 Well-Being Incentive Credits
To earn these well-being incentive credits, complete the following actions between January 15, 2015 and December 15, 2015:
1. Assess your health.
Complete your 2015 Healthways Well-Being Assessment® (WBA), a confidential, online questionnaire that will take about 20 minutes. Please note: the Well-Being Assessment must be completed before well-being incentive credits can be earned.
2. Know your numbers.
Complete a 2015 biometric screening and submit your results (body mass index, blood pressure, cholesterol, glucose). The biometric screening must be completed at an SHBP-sponsored screening event or by your physician and your results submitted appropriately on the 2015 Physician Screening Form. By completing both actions, “Assess your health” and “Know your numbers,” BCBSGa and UnitedHealthcare plan members and covered spouses can each earn up to 240 well-being incentive credits. Please note: the Well-Being Assessment must be completed before well-being incentive credits can be earned.
3. Take action.
It’s your choice! Complete the coaching pathway, online pathway, or a combination of both. By completing the healthy actions outlined in “Take action,” BCBSGa and UnitedHealthcare plan members and covered spouses can each earn up to 240 well-being incentive credits. Please note: the Well-Being Assessment must be completed before well-being incentive credits can be earned.
- Complete your WBA and;
- Actively engage in telephonic coaching.
- Complete your WBA and;
- Record 5 online well-being activities using the same tracker within 4 consecutive weeks and earn 40 well-being incentive credits. You can earn these well-being incentive credits up to 6 times. Sample activities: track exercise five times, record daily steps five times, track food five times and earn 40 well-being incentive credits for your account.
The total 2015 well-being incentive credits earned by completing the above steps may be up to a maximum of 480 for each member and spouse, if covered. Between February 15, 2015 and January 31, 2016 you and your spouse (if covered) may appeal the total well-being incentive credits applied if the rewards are less than you believe should have been awarded to you or your spouse. You or your covered spouse may appeal the total 2014 Health Reimbursement Arrangement (HRA) dollars applied if the HRA dollars are less than what you believe should have been awarded to you or your spouse beginning January 1, 2014. Click here for the process for submitting these appeals.

Wellness 2015 for Kaiser Permanente members and covered spouses
State Health Benefit Plan (SHBP) is excited to partner with Kaiser Permanente (KP). They offer a comprehensive and integrated team approach to wellness. In addition, they provide a variety of wellness tools, resources, and an incentive program designed to empower you to take an active role in your own health. You will have access to KP’s tools, activities, and services such as a total health assessment, biometric screenings, online and onsite healthy living classes.
As part of the KP Wellness Program, you are eligible to win a package, which includes a tablet and a Fitbit®, worth up to $599! All you have to do is complete the wellness activities below to be entered into our monthly drawing, starting January 1, 2015. Each of the seven wellness activities completed between 1/1/2015 and 11/30/2015 earns you one entry; complete them all for more chances to win.
Note: Activities that meet the clinical guidelines and have been performed prior to the start of the program on January 1, 2015, will appear with a green check mark indicating that they are complete. However, these activities completed prior to January 1, 2015, will not count for entries into drawings.
Drawings will be held monthly and 400 packages will be given away during the plan year. Each time you complete a wellness activity, your name will be entered in the drawing, starting January 1, 2015, through November 30, 2015. The last drawing will be held on December 21, 2015. Your entry will remain in the drawing until the end of the plan year or until your name is drawn.
Each activity you complete between 1/1/2015 and 11/30/2015 earns you one entry, sign up for my.kp.org/shbp to get started today.
