IRS Form 1095 Guidance

Per the IRS website, the deadline for insurers, other coverage providers and certain employers to provide forms 1095-B and 1095-C to individuals has been extended to March 2, 2017 for the 2016 tax year. 

State agencies and SHBP retirees (includes SHBP surviving spouses and/or dependents) enrolled in commercial plan options, and SHBP Cobra and state extended coverage participants: 

Individuals who in 2016: 1) were employed with state agencies, 2) were SHBP retirees (including spouse and/or dependents) enrolled in commercial plan options, or 3) were SHBP COBRA and state extended coverage participants will receive their Form 1095s from the State of Georgia.   • Agencies that use TeamWorks HR and TeamWorks Payroll and SHBP
  • DFACS

Board of Education Employees: 

Active Employees who in 2016 were employed with Boards of Education will receive their Form 1095s from their Human Resources Department. 

Members Enrolled in Kaiser Permanente: 

Active employees who in 2016 were enrolled in the Kaiser Permanente HMO Plan Option will receive their Form 1095s from Kaiser Permanente.

Members Enrolled in UnitedHealthCare Medicare Advantage 

Members who in 2016 were enrolled in a UnitedHealthcare MA Plan Option will receive their Form 1095s from UnitedHealthcare.
