ICTF Addendum
Hospitals which participated in the Indigent Care Trust Fund for their Hospital Fiscal Year 2006 (HFYs ending during Calendar Year 2006) must file the 2006 Indigent Care Trust Fund Addendum in order to complete requirements associated with the 2006 Hospital Financial Survey. The 2006 ICTF Addendum was due on or before April 18, 2008.
To Download and Complete the 2006 ICTF Addendum
The 2006 Indigent Care Trust Fund Addendum can be filed using a Microsoft Access database. The survey database should be downloaded, completed, and then electronically submitted by hospitals participating in the Indigent Care Trust Fund during their Hospital Fiscal Year 2006. Click the link below to download the survey database to your computer. Microsoft Access 2000 or a later version is required to open the database and complete the survey.
Please read the instructions below before downloading the survey database. The "Frequently Asked Questions" document will be updated periodically as new questions arise. Please Note: After you click the link below, select "Open" or "Run" when prompted, not "Save". A setup program will then run, which will download the database to the C:\DCH folder on your hard drive.
