Nursing Home Survey
The 2007 Annual Nursing Home Questionnaire (ANHQ) was due on or before August 15, 2007.
The 2008 ANHQ will be released for providers to complete on or around August 15, 2008. The due date will follow the release date by 30 to 45 days.
To Download the Database and Complete the 2007 ANHQ
The 2007 Annual Nursing Home Questionnaire (ANHQ) is a Microsoft Access database. The survey database should be downloaded, completed, and then electronically submitted to the Division of Health Planning by providers of Open Heart Surgery and Cardiac Catheterization services. Click the link below to download the appropriate survey database to your computer. Microsoft Access 2000 or a later version is required to open the database and complete the survey.
Please read the instructions below before downloading the survey database. Instructions are also available within the Access database. The "Frequently Asked Questions" document will be updated periodically as new questions arise.
