Hospital Financial Survey
The 2006 Hospital Financial Survey was due for hospitals to complete on or before March 14, 2008.
The 2007 HFS is expected to be available for completion by June of 2008.
To Download the Database and Complete the 2006 HFS
The 2006 Hospital Financial Survey (HFS) is a Microsoft Access database. The survey database should be downloaded, completed, and then electronically submitted to the Division of Health Planning by hospitals with fiscal years ending in 2005. Click the link below to download the survey database to your computer. Microsoft Access 2000 or a later version is required to open the database and complete the survey.
Please read the instructions below before downloading the survey database. The "Frequently Asked Questions" document will be updated periodically as new questions arise.
