Positron Emission Tomography Services Survey
2008 and 2007 PETSS
Providers owning and operating a Positron Emission Tomography scanner can access and complete the 2007 and/or 2008 survey by clicking here. The 2007 PETSS was due on or before January 16, 2009. The 2008 PETSS was due on or before August 7, 2009.
2006 PETSS
The 2006 PETSS was due on or before July 20, 2007.
Download the Database
The Positron Emission Tomography Services Survey (PETSS) is a Microsoft Access database. The survey database should be downloaded, completed, and then electronically submitted to the Division of Health Planning by providers of Radiation Therapy services. Click the link below to download the survey database to your computer. Microsoft Access 2000 or a later version is required to open the database and complete the survey.
Please read the instructions below before downloading the survey database. The "Frequently Asked Questions" document will be updated periodically as new questions arise.

Use the “Upload” button on the survey opening screen to submit the database. If you have any questions about downloading, completing, or submitting the survey, please contact DCH.
Please Note: After you click the link below, select “Open” or “Run” when prompted, not “Save”. A setup program will then run, which will download the database to the C:\DCH folder on your hard drive.
