2007 OHSS and CCSS
Providers of Open Heart Surgery and/or Cardiac Catheterizations can access and complete the 2007 surveys by clicking here. The 2007 OHSS and CCSS are due on or before October 3, 2008.
2006 OHSS and CCSS
The 2006 OHSS and CCSS were due April 13, 2007.
The 2006 Open-Heart Surgery Services and 2006 Cardiac Catheterization Services Surveys are Microsoft Access databases. The survey databases should be downloaded, completed, and then electronically submitted to the Division of Health Planning by providers of Open Heart Surgery and Cardiac Catheterization services. Click the link below to download the appropriate survey database to your computer. Microsoft Access 2000 or a later version is required to open the database and complete the survey.
Please read the instructions below before downloading the survey databases. The "Frequently Asked Questions" document will be updated periodically as new questions arise.
