Annual Hospital Questionnaire and Addenda
The 2006 AHQ and Addenda was due for hospitals to complete on or before February 29, 2008.
The 2007 AHQ and Addenda will be available for completion by June of 2008.
Download the 2006 Database to Complete the Survey
The 2006 Annual Hospital Questionnaire and Addenda (AHQ) is a Microsoft Access database. The survey database should be downloaded, completed, and then electronically submitted to the Division of Health Planning. Click the link below to download the survey database to your computer. Microsoft Access 2000 or a later version is required to open the database and complete the survey.
Please read the instructions below before downloading the survey database. The "Frequently Asked Questions" document will be updated periodically as new questions arise.

Please Note: After you click the link below, select “Open” or “Run” when prompted, not “Save”. A setup program will then run, which will download the database to the C:\DCH folder on your hard drive.
