Health Planning Databases

The following databases are available for download and use by Division of Health Planning database customers.  The databases are contained within WinZip files and a password is required in order to open and/or extract each database.  Please contact the Division of Health Planning to order a database or password using the form below.  Also, please refer to the instructions for using DHP databases.
