2008 Campaign Details

The Georgia Department of Community Health (DCH) is excited to introduce its “Georgia’s Nuts About Health” 2008 wellness initiative. This campaign is a part of a multi-year commitment to improving your health.

The campaign is designed to assist State Health Benefit Plan (SHBP) members in achieving their individual health, fitness and nutritional goals.

SHBP members who complete a health assessment and one online coaching module will be entered in a drawing to win 1 of 600 Nike+ Runner gift packs.

BlueCross BlueShield of Georgia, Inc (BCBSGa), Kaiser Permanente and UnitedHealthcare (UHC) will maintain the confidentiality of all the data entered on their respective Web sites, and will conduct the drawings and announce the winners in December 2008. Neither the SHBP nor your employer will see any personal information that you provide in response to the health assessment and online health coaching modules.

In order to participate in the 2008 end-of-year drawing, you must follow and complete both steps outlined below on your respective health care vendor’s Web site. All employees, retirees and dependent spouses covered under the SHBP are eligible to participate. However, dependent children are not eligible to participate.

1. Health Assessments

All completed health assessments are automatically recorded for UHC and Kaiser members.

2. Online Health Coaching Modules

All completed online coaching modules are automatically recorded for UHC and Kaiser members.

All completed online coaching modules for BCBSGa HMO or Lumenos HRA options require members to print out a confirmation proving participation in one of these modules and fax it to: 706-494-8600.

Members who have already completed an online health coaching module in 2008 prior to this campaign announcement are not required to complete another health coaching module and will be eligible for the drawing. If you are participating in the BCBSGa HMO or Lumenos HRA options and completed an online health coaching module earlier this year, you will need to print out the proof of completion and fax to (706) 494-8600 or mail to: BlueCross Blue Shield of Georgia, Attn: Barbara Archie or Kellie Kirven, Personal and Confidential, P.O. *Note: Some of the online coaching modules offered by the vendors may take up to six weeks to complete all of the steps, so please allow enough time to meet the campaign deadline.

Winners will be contacted in December 2008 and asked to participate in the Health Nuts group. The Nike+ Program will allow the Health Nuts group to track individual progress and that of the group. The group’s progress will be periodically reported on and there will be awards for those who logged the most exercise. There will be no requirements or pressure for you to exercise above your own pace and your identity can be disguised when you select your user name. Participation in the Health Nuts is an exciting way to see the benefits of regular exercise and compare your exercise levels against the other members of the group. The Georgia Department of Community Health will host an end of year (2009) recognition in Atlanta for the Health Nuts.
