Click here www.kp.org to access the Kaiser Web site or read the following instructions on how to access the health assessment and wellness information. 

1. Log on to www.kp.org
2. You will need to register with a user ID and password 
3. Once the new page opens, go to “My Health Manager” to access the
   Total Health Assessment
4. Click on “Total Health Assessment”
5. Under “Your Available Programs,” click on “Health Media Succeed”

In addition to completing the health assessment, you must return to the “Health and Wellness” tab and enroll in one of the Healthy Lifestyle programs such as Lose Weight, Eat Healthy, Reduce Stress, etc.

Note:  You will not be eligible to participate in the drawing if you do not follow the above instructions.
