- Log on to www.myuhc.com
- Click on “Site Login” and enter username and password or “Need a username and password” if a first time user
- Click on ”Health Assesment" tab
- Read the privacy information and then click on "Launch University of Michigan Health Assessment”
- Answer the questions and hit "Submit to the
- Review your personal results profile. Your completed health assessment
will personalize your online health and wellness experience
In addition to completing the health assessment, you must click on the “Online Health Coach” and participate in one of the coaching modules, such as weight loss, nutrition, stress management, etc. Please note some programs may take several weeks to complete.
Note: You will not be eligible to participate in the drawing if you do not follow the above instructions.
