Click here www.bcbsga.com to access the BCBSGa HMO Web site or read the following instructions on how to access the health assessment and wellness information.

1. Log on to www.bcbsga.com
2. Click on “Register” or “Login”
3. Click on “360° Health Program” and complete “My Health Assessment”
4. Click “Healthy Living” and select a program under the Tools and
   Information box.  Some of the modules may take five to six weeks
   to complete
5. Once you complete the “My Health Assessment” and one of the online
   modules, you can fax your confirmation materials to (706) 494-8600 or
   mail to:

Important: “My Health Assessment” results are confidential and should only be shared with your doctor.  Please do not print and send your health assessment.  You should only send the proof of completion, listed below.

For proof of completion, print one of the following:

• Nutrition - print the congratulations page on your food diary
• Ready, Set, Stop - after you finish the program print your confirmation
• Fitness (LEAP) - print the activity log

Note:  You will not be eligible to participate in the drawing if you have not followed BCBSGa instructions.

BCBSGa Lumenos HRA

Click here www.lumenos.com to access the Lumenos Web site or read the following instructions on how to access the health assessment and wellness information.

1. Log on to www.lumenos.com
2. Click on “I am enrolled” or “I want to learn more”
3. Enter username and password
4. From the Main Page under “Stay Healthy” click on “My e-Checkup”
5. Complete “My e-Checkup”
6. You are finished when you see your Health Quotient (HQ) score

In addition to completing the e-Checkup, you must also click on “Health Channels” and select one of BCBSGa health modules.  Some of the modules may take five to six weeks to complete.  Once you complete the module, print and complete the Participant Submission Form.

Important: My e-Checkup results are confidential and should only be shared with your doctor.  Please do not print and send your e-Checkup.   You should only fax or mail the Participant Submission Form to BCBSGa.  Fax # (706) 494-8600.
