Notice to State Health Benefit Plan Members Enrolled in Blue Choice of Georgia

The State Health Benefit Plan (SHBP) has learned that personal data of some SHBP members enrolled in the Blue Choice HMO was mailed to other members in error, due to a computer programming error by Blue Cross and Blue Shield of Georgia (BCBSGa).

This data may have contained members’ personal identifying information such as names, health care services received, member identification numbers, Social Security numbers, or other health plan related information. 

SHBP has taken the following actions:

• SHBP will provide all members who may be affected with written notice.
• SHBP will provide all members who may be affected with information about free credit reports and the availability of credit freezes.  
• SHBP will provide all members who may be affected with a resource to contact for additional information by calling BCBSGa at the toll free number below.

All members will receive notice by mail and may call BCBSGa at (866) 800-8776 Monday through Friday from 7:00 a.m. to 9:00 p.m. for more information.
