New Pharmacy Benefit Manager for SHBP PPO and Indemnity members

Effective January 1, 2008, UnitedHealthcare became the new pharmacy benefit manager for PPO, PPO Choice and Indemnity members covered by SHBP.  If you have questions about  2007 pharmacy claims - call ESI Customer Service at (877) 650-9342.  For 2008 claim or pharmacy benefits - call UnitedHealthcare Customer Service at (800) 372-5802.
